Privacy policy
Confidentiality and personal data security policy. Information about the processing of personal data (as Controller and Processor, under the EU General Data Protection Regulation 2016/679).
1. Why Fertility Clinic processes the personal data of patients
Fertility Clinic processes personal data in order to achieve a positive outcome in the treatment of infertility, and to ensure identification (traceability) between samples and the patient. Fertility Clinic collects data in order to operate effectively and to provide you with the best IVF services. Data are collected for specified, explicit and legitimate purposes and are not further processed in a way that is incompatible with those purposes. Data are kept in a form that allows identification only for as long as is necessary for the purposes of the processing.
Providing accurate and complete data is very important for the safety, quality and effectiveness of the results we provide. If the information you give us is incomplete or inaccurate, this may affect our ability to provide you with safe and effective care.
2. The data we collect
- Name and contact details: first name and surname, email address, postal address, city and telephone number, in order to identify the patient and the sample, and to keep the patient informed, for example through a newsletter.
- Demographic data: age and sex, in order to identify the patient and the sample.
- Medical history and test results, in order to process and analyse the sample correctly.
- Financial data: tax number, health insurance (including social security number, AMKA), any private health cover, and referrals from the Greek national health service (EOPYY).
3. How Fertility Clinic processes my data
Once a patient has provided their data to Fertility Clinic, authorised staff will, for the purposes described above, carry out any operation or set of operations on the data, including with automated or semi-automated means, such as collection, recording, organisation, correction, storage, adaptation, alteration, retrieval and consultation. These processes allow Fertility Clinic to make decisions more quickly, accurately, transparently and consistently.
4. How long Fertility Clinic keeps patient records
Records are kept for ten years, both on paper and electronically. Fertility Clinic complies with Greek law on the retention of medical records.
5. Your rights regarding the processing of your data
Depending on the case, you may exercise the following rights: the right of access (to find out which of your data we process, why, and who receives them), rectification (to correct any gaps or inaccuracies in the data you provided), erasure or the “right to be forgotten” (deletion from Fertility Clinic’s records, provided the processing is no longer necessary), restriction of processing (for example if the accuracy of the data is disputed) and data portability (to receive your data in a structured, commonly used format).
You can exercise these rights free of charge by sending a letter to the clinic’s address (End of Ariadnis Street, Eleones, Pylaia, 555 35 Thessaloniki, Greece) or an email to info@fertilityclinic.gr, for the attention of the Data Protection Officer. When you exercise any of these rights, Fertility Clinic will take every possible measure to meet your request within thirty (30) days of receiving it, informing you either that it has been met or of the objective reasons that prevent this.
In addition, you may at any time object to the processing of your personal data for the purposes of the service agreement by withdrawing your consent. However, this will end the provision of services to you, because no service can be provided without processing your personal data.
6. This website and its forms
This website sets no cookies and loads nothing from other websites while you read it. Requests you send with the website’s forms are emailed by the website’s own server straight to the clinic’s mailbox (info@fertilityclinic.gr), without any separate form service; the website itself does not store them. The appointment form sends your name, phone, email (if you give it), how you would like us to contact you, the topic, your preferred days and times or month, whether you are travelling, your message, your consent and the language of the page; the “Shall we call you?” form sends your name, your phone, your consent, the page you sent it from and its language. We use them only to contact you about your request.
The map on the contact page is loaded from OpenStreetMap only if you press “Show map”; OpenStreetMap then receives your IP address.
7. Complaints
If you believe that your personal data has not been handled correctly, you can lodge a complaint with the Hellenic Data Protection Authority (Αρχή Προστασίας Δεδομένων Προσωπικού Χαρακτήρα), www.dpa.gr.
